Application for Organizational Membership (New or Renewal)

This named organization hereby applies for organizational membership in the Ohio Cave Survey, Inc.  We understand that membership guarantees no specific access to data or maps.  We agree to abide by the bylaws and goals of the OCS and fully understand that any actions violating these rules may result in dismissal from the Ohio Cave Survey, Inc.

If you do not receive a reply about your approved membership within seven business days,
send an e-mail to Treasurer Gary Bush and he'll check on it.

You can use Zeffy to pay the appropriate dues, using a major credit card
- or -   if you'd rather, mail a check with the paper form to the OCS Treasurer.
(NOTE: We have switched from Paypal to Zeffy to ensure all of your dues go to OCS.)

Note Changes with Using Zeffy:

Zeffy’s service is free to OCS.   PayPal, Square, and other services typically charge 3 to 5% to OCS. Zeffy supports itself through an optional tip (to themselves). Zeffy’s website adds this tip onto your OCS dues. That tip money goes to Zeffy – Not to OCS. You can change that amount. In the Summary section, click the arrow to the right of their suggested 17% amount and select "Other", then enter whatever tip you feel is fair.

This tip is your choice, so it's optional, but you must change it or their initial suggested amount (17%) will be added. It is not refundable.

Additionally, you will have the option with Zeffy to make your dues Auto-Renew each year. That way, you won't forget to renew every year. You can cancel auto-renew at any time.
