I hereby apply for membership in the Ohio Cave Survey. I understand that membership guarantees no specific access to data or maps. I agree to abide by the bylaws and goals of the OCS and fully understand that any actions violating these rules may result in my dismissal from the organization.

After you click on the "Submit Request" button, click the "Reset the Form" button if your information does not clear from the boxes. Then, pay your dues in the box below.
If you do not receive a reply about your approved membership within seven business days, send an e-mail to Secretary Sarah Lott and she'll check on it.

 

After completing the form above, you can use Paypal to pay the appropriate dues,
using either your Paypal account, if you have one, or simply a major credit card ...
- or -   if you'd rather, mail a check to the OCS Treasurer.
